It depends on your authoring program, however for me I'm using ConvertXtoDVD
which always sets the flag of the menu (only) at 4:3

What I do, as I'm using a 16:9 display, make sure my menu image usually a
bmp or jpg file is in real terms an image with a 16:9 aspect ratio I then
squeeze it to 4:3 in PaintShop Pro. I then edit in IFOEdit the menu from 4:3
to 16:9 and that works for me - if you are using a videoclip except for the
menu (can't do that in ConvertXtoDVD) then it would be an even more
circuitous route!

What i've seen in some dvd menu's is, use a widescreen menu picture
format, keep all buttons and text within the 4:3 frame borders, and set
the menu flags (ifoedit) for pan&scan, this way aspect ratio of the
picture geometry will not be affected on any playback screen,
For the menu-item you don't want to use a letterbox option, for
movie/title item you can, because on a 4:3 screen the movie is then
default letterboxed, on a 16:9 screen the movie/title will be
filled-outon the screen in it's correct ratio, unless the viewer
switches manualy, or has set it's prefs of his dvd player....
