2) Fill the form.

This gives you 1 month free of the 3-movies out at a time plan. They
carry over 60,000 movie titles and you can return them via mail (free
postage) or in the store. If you return it in the store they give you a
store rental for free right then and there. This program called
Blockbuster Total Access.

Total Access means you've got FREE, three ways:
DVD mailers good for FREE in-store movie rentals
E-Coupons good for FREE in-store movie rentals OR...
Now, E-coupons are good for FREE game rentals as well.

It's not free if they charge your credit card $50 and you can't cancel
out of a membership which charges you $25 a month.

They ask for credit card in case if you loose the dvd's. If you cancel
after 1 month free trail they are not going to charge you anything.
But do not forget to cancel before the free trail ends.
