error: the video device failed to initialize for fullscreen mode

Happens on character based apps such as cmd.exe.

I'm not using terminal server.

xp w/sp2 512mb ram athlon xp 1900+
nvidia ti500
using the microsoft nvidia drivers from sp2
same happens using nvidia's drivers

booting into safe and vga modes does the same

Ideas?
